Continuing Education form templates
All templates are locked forms, created in Word. Information for a given CE event can be entered in the form fields without unlocking it; shading will not show when the form is printed.
To edit a form, unlock it, using the Developer tab on the ribbon. If you don't see that tab, you can add it. While in the document, go to File/Options/Customize the Ribbon, then look under 'Main Tabs' and check off the 'Developer' box. Once the Developer tab is available, click on it, and use 'Restrict Editing' to unlock the form. The forms were built as tables, with the lines hidden; use 'show/hide ¶' in the 'Home' tab to see the formatting. MS Word's built-in help function has instructions.
